Student transition from being a full time student to being out on their own is a critical time. Quite often students do not know what they should do or even want to do when they complete. This can cause them to be filled with anxiety, confusion and even hopelessness. Centers have developed evaluation forms to walk students through the key components of a healthy transition. The counselor works with the student to ensure there is a legitimate way to meet these critical needs. Attached are forms available to guide this process. These forms cover areas such as:

  • Employment
  • College / Enrollment
  • Church home
  • Support group
  • Mentor – Accountability partner
  • Legal issues
  • Living arrangements
